What Is An Apicoectomy?

Dr. Amit Pandya performs many root canals to save and repair teeth that are badly damaged or infected.  During a root canal a dentist removes any inflamed or infected tissue, and thoroughly cleans the root canal system. In about 90% of cases, the tooth heals and doesn’t require further treatment. When an infection won’t go away or the canals become re-infected after root canal treatment, Dr. Amit Pandya can perform an apicoectomy.


An apicoectomy is also called a root-end resection and is performed to remove damaged tissue and seal a tooth’s root from any further infections.  The procedure involves a small incision in the gums, followed by removal of any infected tissue and finally the end of the root tip itself.  A root-end filling is placed there to act as a seal, preventing reinfection. Pediatric Dentistry

Dr. Amit Pandya is proud to serve the families from our community, and we want them to know it’s just as important for children to have regular dental visits at our practice.  A child’s first dental visit should be scheduled between when the first baby tooth appears, and the child’s first birthday.  Baby teeth eventually come out, but their growth and development is important for overall oral health for the child and the development of their adult teeth. Flossing with Traditional Braces

Flossing with traditional braces may seem impossible, but it’s not. If you don’t floss between the brackets and wires, you will develop periodontal disease which will make your gums sensitive, red, and swollen. Dr. Amit Pandya, DDS, will guide you through the process of flossing with traditional braces, but you can start with this simple technique.



  • Use waxed floss, because unwaxed floss is more likely to get caught and shred in your braces.

  • Use about 18 inches of floss and thread it carefully.

  • Take the floss and gently thread it under the main wire of the braces before passing it between two teeth. Then remove the floss and re-thread it under the main wire to pass between the next pair of teeth.

  • Be sure not to snap the floss. Simply move the floss up and down carefully against the side of each tooth.

Inlays and Onlays

When patients chip a tooth or have extensive decay, we can help them at Dr. Amit Pandya’s New Hyde Park dental office. Replacing an entire crown is not always necessary—we also offer partial crowns, known as inlays and onlays.


The chewing surfaces of the molars and premolars have raised cusps with indentations between them. These cusps can sometimes chip and food debris can get stuck in the indentations, eventually leading to decay. When that happens, we’ll drill away the decayed section and then make an impression of the patient’s mouth. The patient is provided with a temporary artificial crown while the impression is used at a dental lab to manufacture a prosthetic. An onlay is a partial crown that replaces or covers a cusp; an inlay is one that settles between them.


Once the partial crown arrives at our office, we’ll be able to cement it into the patient’s tooth. The patient should then be free to enjoy their restored bite function. A partial crown still has to be brushed regularly. Patients can also prevent decay by getting fluoride sealants to smooth out their teeth’s indentations.

Reasons for Extractions

When we extract teeth at Dr. Amit Pandya’s New Hyde Park dental practice, we do so quickly after ensuring that our patients fully understand their post-operative instructions. But because the outcome of extractions depends so heavily on the patient’s commitment, we want to make sure they are aware of why they are undergoing extractions in the first place and why it is often the best option.


A lot of extractions in young people are done to ensure the remaining teeth will have enough room to form a healthy arch. Well-aligned teeth are easier to keep clean, won’t put as much strain on the jaw muscles during chewing, and keep the face straighter, but the mouth often doesn’t have enough room for all of them. A set of premolars are often removed prior to orthodontic work, and wisdom teeth are commonly extracted to prevent them from getting stuck while erupting. Baby teeth may also be extracted if they do not fall out naturally before their adult replacements grow in.


We always try to save teeth that have been damaged, but sometimes extractions are necessary to protect the patient’s health. A cracked crown can be repaired or replaced, but a crack that goes all the way through the crown and root cannot be and the tooth will have to be replaced with an implant.

Rheumatoid Arthritis and Gum Disease

Autoimmune disorders often complicate peoples’ oral health, and rheumatoid arthritis (RA) is no exception. Scientists have recently made great strides in understanding how this disease affects the gums, and we want patients at Dr. Amit Pandya’s dental practice in New Hyde Park to know what they’re facing.


People with RA have extremely high rates of gum disease. A recent study by Johns Hopkins found the number could be as high as 70%. While scientists formerly believed this was because people with RA have difficulty moving their hands, they noticed that people whose RA was still in its early stages also had high rates of gingivitis.


New research has found similarities between gum tissue and synovium, the joint lining that becomes inflamed when the immune system mistakenly attacks it. Some of the medications that are used to combat RA may also cause dry mouth, which increases the risk of gingivitis. Patients may need an electric toothbrush to maintain their oral hygiene, as well as throat moisturizers and water picks. They can also reduce their risk of RA and other oral health diseases by not smoking.

Types of Teeth

Every tooth needs to be brushed and flossed to stay healthy. But the teeth also have specialized functions. At Dr. Amit Pandya’s New Hyde Park dental practice, we believe patients should have an understanding of basic oral health issues, which is why we wanted to provide a guide to what makes teeth distinct.


Adults typically grow thirty-two permanent teeth to replace the twenty-two they grew in childhood. There are sixteen teeth in each jaw. The four in the front of the jaw are called incisors, and they are used for speech and for tearing food. To the sides of the incisors are the cuspids, so called because they end in a point. They are also called canines or eye teeth. The upper cuspids sometimes have difficulty breaking through the gum line, and in rarer cases, so do the lateral incisors.


The premolars and molars are used for chewing. Their crowns have multiple cusps, but the premolars (four per jaw) only have one root. The molars (three per jaw) in the lower jaw have two roots and the ones in the upper jaw have three. Molars have strong ligaments and have to withstand incredible force. The wisdom teeth, which are the largest molars and the ones furthest in the back, also commonly have difficulty erupting.

Aggressive Periodontitis

If you’ve been told by Dr. Amit Pandya, DDS that you have gingivitis, you should take treatment seriously, as gingivitis is reversible with good oral habits. If left untreated, gingivitis will progress to a more aggressive form of gum disease called periodontitis. Periodontitis means inflammation around the tooth. When inflammation becomes aggressive, this can destroy the surrounding bone, potentially leading to tooth loss.


Periodontitis develops when bacteria and plaque build-up around the tooth, and the immune system launches a reaction. Periodontal disease in any form can be prevented by brushing at least twice a day for two minutes and flossing at least once a day every day. To restore periodontal health, Dr. Pandya may suggest scaling and root planing treatment to clean underneath the gums and remove harmful bacteria.

Causes of Bleeding Gums

When patients have bleeding gums, Dr. Amit Pandya of New Hyde Park is here to help. Unfortunately, many people are not even aware that bleeding gums are a problem, and think that seeing blood when they brush their teeth is normal. Perhaps they would think otherwise if they knew what they are seeing is caused by an infection.


Having some bacteria in our mouths is necessary for our health, but bacteria grow in excessive numbers when there is food debris available to them. Bacteria produce acid as a by-product of their metabolism, and the acid gets stuck in the sticky plaque surrounding teeth. Plaque in gum pockets is hard to remove, so bacteria often grow there, and the resulting acid damages gum tissue.


When gums first become inflamed, they are said to be suffering from “gingivitis.” If the infection becomes more severe, a patient develops “periodontal disease.” At this point, their gums will not only bleed, but recede, imperiling the stability of the teeth. Patients can prevent this by brushing their teeth thoroughly, but it is still a good idea to regularly come to our office for deep cleanings.

Intrinsic Staining

As part of his quest to provide optimal dentistry, Dr. Amit Pandya of New Hyde Park helps people with stained teeth to brighten up their smiles. The most common way of doing this is with chairside whitening, which leaves patients with teeth that are several shades lighter for months. But some stains, those which are intrinsic to the tooth, require a more involved approach.


Usually, stains are acquired from things consumed in adulthood, such as smoke or coffee. These can be bleached away. But intrinsic stains may be the result of something the tooth was exposed to during its formation, such as excessive amounts of fluoride or tetracycline antibiotics. These stains are too deep to be bleached, but they are not a health hazard and can be covered with veneers.


Intrinsic stains resulting from infection are another matter. An infection in the dental pulp may not be painful initially, but it is likely to become so and can imperil the patient’s life if it spreads to the bloodstream. A root canal will be done to remove infected pulp, after which the tooth may be given a new crown.

Sectioning Teeth

At the New Hyde Park dental office of Dr. Amit Pandya, we understand the concerns people have about tooth extractions. Natural teeth are always the best and extractions should only be done when necessary. However, when impacted wisdom teeth threaten the health of the rest of the teeth, we remove them quickly and with minimal risk to the patient. This sometimes requires us to section the teeth before removal.


Wisdom teeth are the largest teeth and have multiple roots attached to strong ligaments. Trying to remove them in one piece could subject the patient to a dangerous amount of force even if the wisdom teeth were in a perfect position which, if they are being removed, they probably are not. Impacted wisdom teeth are often located close to major nerves and are too large to move without cutting away some gum tissue.


Teeth can be moved with less force and through smaller gum flaps when they have been cut apart. This is only done after the patient has been numbed and sedated. A dental drill is used to do the cutting and the surgical site is kept under a stream of water to prevent damage from heat. Sectioned teeth can be lifted out of the jaw easily and the gums can then be sutured.

Childhood Halitosis

We help patients of all ages with all sorts of oral health needs at the New Hyde Park dental office of Dr. Amit Pandya. Adults aren’t the only people who get bad breath, and when children get it, there may be some specific issues which need to be addressed for their oral health as well as their social prospects.


Halitosis, as bad breath is scientifically called, is the result of excessive bacterial growth in the mouth and throat. There may be some condition which is allowing bacteria to grow or preventing the body from fighting infection, but in most cases, the problem is simple lack of hygiene. Children sometimes have difficulty brushing every tooth surface and on their tongues for a full two minutes. These children will require more teaching and supervision and may benefit from the use of an electric toothbrush.


The most common condition which would contribute to halitosis is dry mouth. Besides making it harder to remove food debris, dry mouth disrupts the mouth’s optimal acidity level, hampering the immune system. This could be caused by diabetes or medications, such as those commonly used for psychiatric conditions. Children are also particularly prone to nasal infections, which could result in drip accumulating in their throats and feeding bacteria. Non-alcoholic mouthwash could help with this problem.

Bad Oral Habits

Most of us have habits that we aren’t proud of. When these habits begin to affect the teeth, Dr. Amit Pandya, DDS encourages you to break them immediately. Bad oral habits such as these can cause fractures in the teeth, may leave gums and enamel feeling sensitive and may lead to costly dental treatment in the future. If you practice any of these bad oral habits, do your best to reduce and eliminate them.



  • You’re brushing too hard – over time, if you brush with too much force, you can wear down the protective layers of your teeth and expose the microscopic hollow tubes or canals that lead to your dental nerves. When these tubules are exposed, tooth sensitivity is a result.

  • You’re eating too many acidic foods – If the pathways to your nerves are exposed, acidic foods will leave you in pain.

  • You have tooth decay – If you have tooth decay, it’s easy for bacteria to accumulate in these tiny crevices, which causes acid build-up and enamel breakdown.

  • You’re a tooth grinder – Grinding your teeth can wear down your enamel. This exposes the dentin and causes sensitive teeth.

Tooth Sensitivity

Sensitive teeth can be very uncomfortable. A patient who has fell victim may feel gum inflammation and possibly wince at the thought of drinking a hot or cold beverage. Possible causes of sensitive teeth include tooth decay, worn fillings, and periodontal disease. However, the most common reason for tooth sensitivity is a poor oral care routine. In healthy teeth, a layer of enamel protects the crowns of the teeth – the part above the gum line. If proper oral care isn’t practiced, gums may recede and this will cause sensitivity. Treatment options for sensitive teeth may include desensitizing toothpaste, fluoride gel, a gum graft, or a root canal depending on the severity of the sensitivity. To prevent sensitive teeth you should always brush at least twice a day for two minutes and floss at least once a day.

What is a Bone Graft?

A bone graft is the term for harvesting bone material from a healthy location and grafting it to a location that is broken or weakened.  Dr. Amit Pandya performs bone grafts on patients in need of dental implants.  Missing a tooth for an extended period of time can leave the jaw bone vulnerable and compromise its structure.  When a patient needs an implant for a permanent replacement tooth, a bone graft reinforces the jaw bone so it is strong enough to handle the placement of an implant. 


In the same procedure, bone material is harvested from a healthy location in the patient’s mouth and grafted to the compromised area of the jaw bone.  This strengthens the weakened bone, and encourages growth of new bone material as it heals.  After the procedure, the bone graft will be monitored over the course of months.  When the jaw bone has strengthened enough to handle the placement of an implant, the dentist can move forward with the patient’s plan of care.

Temporomandibular Joint Disorder (TMJ/TMD)

Each of us has a temporomandibular joint. This joint is a hinge that connects the jaw to the temporal bones of the skull which lay in front of each ear. Without this joint, we wouldn’t be able to enjoy the foods that we love, yawn when we are tired, or speak. If you’ve ever woken up in the morning with jaw pain, or notice a clicking or popping noise when you chew, Dr. Amit Pandya, DDS may need to evaluate you to rule out a temporomandibular joint disorder.


Though there is no known cause of TMD, specialists have some guesses. An injury to the face, grinding the teeth because of bruxism, or stress on the joint can lead to problems. TMD often causes discomfort and pain and may make eating or speaking seem like a difficult task. Once you have been diagnosed with TMD, Dr. Pandya will likely suggest a mouth guard or a night guard to wear during sleep to prevent further complications.

Invisalign Treatment

If you are an adult who thinks the window of orthodontic treatment has passed, Dr. Amit Pandya, DDS is here to debunk that idea with Invisalign. Invisalign is a teeth-straightening method that uses virtually invisible aligners to shift the teeth slightly, moving them horizontally and vertically, and even rotating them when necessary. Invisalign treats a wide variety of malocclusion problems from overbites to underbites. Caring for Invisalign

Dr. Amit Pandya, DDS has helped transform many smiles with Invisalign, the clear aligner system that straightens teeth without the metallic look of braces.  Other perks of clear aligners are that they require less maintenance and don’t prevent you from eating any particular foods.  Although you won’t have to make too many changes, there are still guidelines you should follow to wear and care for your aligners.  You should remove Invisalign each time you eat, but if you do eat anything while wearing the aligners make sure to clean them thoroughly afterwards. It is also best to brush and floss your teeth before putting the aligners back in.  Ideally when wearing clear aligners, you would only drink water. Colorful drinks can stain the aligners and too many hot drinks can eventually warp the plastic.  If you do drink anything other than water while wearing the aligners you should rinse them with warm water afterwards. 


Cleaning the clear aligners regularly is important for your teeth and straightening treatment.  Clear aligners can become discolored over time without losing their effectiveness, but the color can show on your own teeth and continue to darken.  Invisalign offers its own brand of cleaning solution, and there are several soaking solutions available over the counter, but brushing the aligners is also a sufficient cleaning option.  Do not use toothpaste to brush the aligners because subtle abrasive properties of toothpaste could harm the aligner.  It is best to use mild dish soap and a soft bristle brush to clean the aligners each time you brush your teeth.

Sensitivity with Fillings

At Dr. Amit Pandya’s New Hyde Park Dental practice, patients needing relief from tooth decay are supplied with naturally-colored, expertly crafted fillings. However, there are some situations in which they would require a dentist’s attention, which is why we want to provide our patients with information about what to look out for.

Acid Reflux

At Dr. Amit Pandya’s New Hyde Park practice, we’re committed to preventing and treating dental damage no matter its source. Although tooth decay is caused by bacterial infection, loss of erosion to acid reflux is another problem patients need to be on guard against.


Gastrointestinal reflux disease (GERD) describes when acid escapes from the stomach into the esophagus at least twice a week. The damage the acid does on its way up is commonly, but inaccurately, called heartburn. Acid increases the risk of esophageal cancer and if it gets as far as the teeth, it can cause erosion. Its effects will usually be visible first on the upper back teeth, which will take on a glazed appearance before becoming pitted and yellowish as the underlying pulp shines through.


Acid reflux makes teeth more sensitive but can be guarded against with fluoride treatments. Patients may also reduce their frequency of acid reflux by giving up smoking and by cutting down on citric juices, chocolate, fried food, red meat, caffeine, and foods in the onion and garlic group. It also helps to avoid large meals before bed.

Different Types of Sleep Apnea

Sleep apnea is a potentially serious sleep disorder in which breathing repeatedly stops and starts. There are two main types of sleep apnea: obstructive sleep apnea, the more common form that occurs when throat muscles relax, and central sleep apnea, which occurs when your brain doesn’t send proper signals to the muscles that control breathing. Dr. Amit Pandya, DDS can discuss treatment options if you’ve been diagnosed with this condition, but first, we would like to explain the difference between the two.


When obstructive sleep apnea occurs, the muscles relax, your airway narrows or closes as you breathe in, and you can’t get an adequate breath in. Your brain senses this inability to breathe and briefly rouses you from sleep so you can reopen your airway. You may make a snorting, choking or gasping sound. When central sleep apnea occurs, you may awaken with shortness of breath or have a difficult time getting to sleep or staying asleep. Like with obstructive sleep apnea, snoring and daytime sleepiness can occur.

Lip Care in Cold Weather

Dental health does not exist in isolation; it is connected to that of other body systems. At Dr. Amit Pandya’s New Hyde Park practice, we’ve seen the ravages of the winter weather on patients’ lips and we want them to understand how to protect themselves and why it is relevant to dental care.


The lack of moisture in cold air makes things tougher for people who suffer from dry mouth. When the body has difficulty producing saliva, it is more vulnerable to oral infections, including those which cause tooth decay. Humidifiers placed strategically around homes and workplaces can help combat this, especially when placed where the patient sleeps.


Two things that are definitely not helpful are long showers and moistening the lips with saliva. Wetting the lips causes them to dry out faster due to evaporation. Holding them under the teeth is worse; this will dry them out irritate them, and if done frequently, could cause the teeth to become misaligned. A better option is to use lip balm to seal in moisture, but some ingredients, such as fragrances and menthol, cause some peoples’ lips to become irritated. A change in products is necessary if this is a problem.

The Correct Way to Floss

Plaque can be detrimental to our oral health if not removed with adequate brushing and flossing. This is because plaque is a biofilm, or mass of bacteria that grows on the surface of teeth once we are done eating. Plaque is the main cause of cavities and tooth decay, and to eliminate it, you’ll need to brush at least twice a day and floss at least once a day. To get the most out of your flossing technique, Dr. Amit Pandya, DDS suggests utilizing these tips.  



  • Break off about 18 inches of floss and wind most of it around one of your middle fingers. Wind the remaining floss around the same finger of the opposite hand. This finger will take up the floss as it becomes dirty.

  • Hold the floss tightly between your thumbs and forefingers.

  • Guide the floss between your teeth using a gentle rubbing motion. Never snap the floss into the gums.

  • When the floss reaches the gum line, curve it into a “C” shape against one tooth. Gently slide it into the space between the gum and the tooth.

  • Hold the floss tightly against the tooth. Gently rub the side of the tooth, moving the floss away from the gum with up-and-down motions. Repeat this method for the rest of your teeth and don’t forget the molars!

Proper Brushing Technique

Most of us have been brushing our teeth for years, but you may find it surprising that there are a right way and a wrong way to clean the teeth. Dr. Amit Pandya, DDS wants all patients to practice preventive oral hygiene. This includes brushing at least twice a day for two minutes and flossing at least twice a day. Next time you brush your teeth, try this technique.



  • Place your toothbrush at a 45-degree angle against the gums.

  • Move the brush back and forth gently in short (tooth-wide) strokes.

  • Brush the outer tooth surfaces, the inner tooth surfaces, and the chewing surfaces of the teeth.

  • Use the tip of the brush to clean the inside surfaces of the front teeth, using a gentle up-and-down stroke.

  • Brush your tongue to remove bacteria
